Transition Health Care (THC) is a licensed medical services company with 25 medical professionals and over 120 Certified Nursing Assistants delivering services to independent senior living facilities and assisted living locations 24 hours a day, 7 days a week. We take pride in going “above and beyond” to meet our resident’s needs, serving a subset of 23 Lang Nelson independent senior, assisted and catered living communities. Transitions Health Care provides a range of amenities and care levels so residents can customize their living experience. We have partnerships with primary care physician groups, a spectrum of medical professionals, and specialty service companies. This allows our in-house health care company to offer a complete continuum of care delivery and related services.
Our company focuses on bringing exceptional, caring service to the client rather than have the client commute to an office location. This is only one example of placing residents needs as paramount in our service programs. THC intentionally incorporates the basics of friendly personalized care, with cutting edge systems (i.e. medication delivery and remote encounters with physicians) to deliver the absolute best care
