Kathy Anderson RN, Nurse Liaison with Lang Nelson Associates is happy to introduce our residents and their families to medical and community resources that will enhance their lives.

Growing old is an honor that is not bestowed upon everybody, and there are so many wonderful aspects of aging!

We have a lifetime of wisdom to share with the generations of our families and communities. And while our memory capacity may change; our knowledge, skills, and expertise don’t. In fact our creativity may increase, we may feel more on an even keel emotionally, and instinctively we invest in what is important to us and have the capacity ‘to see the big picture.’

So as we age, we can use the strength of our generations’ work ethic, our wisdom, and our knowledge to help improve our families, and society.

Remember, the gains that come with aging far outweigh the challenges we may face with vision, hearing, various illnesses, and memory changes.

We are fortunate to have an array of resources we can all upon to help us transition through our various life changes.

The Lang Nelson Goal is that you remain comfortable, independent, and safe living within our communities.

Creekside Gables is located in the beautiful Village Creek redeveloped area of Brooklyn Park, Creekside is also an Independent Living Senior Community. Creekside is much more than just a place to live, it’s all about the people that live here and the fun that takes place on site. In talking with our residents here at Creekside Gables we have discovered a lot of hidden talents among our residents. Through a conversation with Jacqueline Webb (resident) I found out that she used to play with the Minnesota Mandolin Orchestra. I talked a little more with Jacqueline and she offered to ask the orchestra if they would come and play for our residents. The orchestra played for Creekside on April 19th 2012. The music was beautiful and the sound was great. Everyone came and enjoyed all the music that was played. The orchestra said they had never been so well received and they would love to come back and play again for us. I have contact information if you would like to contact the orchestra and have them play for your organization. Again, they are outstanding.

Hospice Care

St. Croix Hospice is proud to have been working with Lang Nelson communities since the fall of 2009.  Together our organizations collaborate to bring hope and healing to residents within their homes and community.

The Entitlement Benefit: Paying For Hospice Care

Are you aware that Hospice services are most often covered at no additional expense to the individual patient or family? Hospice is a benefit entitled to those who are eligible for Medicare, Medicaid and other private insurance and allows for additional care to the patient and support to the primary caregiver.

Medicare

For Medicare beneficiaries, hospice services are covered under Medicare Part A. You are eligible for the Medicare hospice benefit when:

• Eligible for Medicare Part A.

• Doctor and hospice medical director certify that you are meet eligibility requirements based on terminal diagnosis and have decided to pursue a palliative, non-curative course of treatment.

• Sign a statement choosing hospice services instead of routine Medicare covered benefits for your terminal illness.

• Receive care from a Medicare approved hospice program.

Medicaid

The state Medicaid program determines your eligibility, which is limited to individuals who meet eligibility requirements based on income, assets and meet general hospice eligibility set forth by Medicare.

For Patients with Other Insurance Coverage

Various types of insurance plans cover many of the professional services offered by St. Croix Hospice. A St. Croix Hospice social worker is available to discuss insurance and assist the patient to receive the maximum available coverage for needed services. St. Croix Hospice will never discontinue services because of a proven inability to pay

St. Croix Hospice brings hospice care to the Twin Cities metro area, as well as Cambridge, St. Cloud, Rochester and Western Wisconsin. Services available through St. Croix Hospice include skilled nursing, music therapy, massage therapy, social work, spiritual care and bereavement care.
