Saint Jude Hospice is proud to have been working in partnership with Lang Nelson communities since the fall of 2009.  Together our organizations collaborate to bring hope and healing to residents within their homes and community.

Saint Jude Hospice is proud to announce its new Pulmonary Program, which is specifically designed to bring additional expertise, resources and optimal comfort to hospice patients with pulmonary disease.  The program focuses on the special needs of patients with pulmonary disease and their families by providing education, training and resources to all involved with patient care.  As a result, the Pulmonary Program can provide improved quality of life and better self-care related to symptom management of pulmonary disease.

The Pulmonary Program, developed with input from Minnesota’s leading pulmonologists, primary care physicians, respiratory therapists, nurses and patients with lung disease, aims to provide the highest quality of life possible with end-stage lung disease.  Saint Jude Hospice also has a respiratory therapist/hospice liaison on staff to lend expertise to the multi-disciplinary care team.

Nurses at Saint Jude Hospice recently participated in a specialized training course related to the pulmonary needs of patients, families, caregivers, physicians and healthcare providers.  Pulmonary education is also available to patients, families, senior living communities, healthcare providers, caregivers and the community.

Saint Jude Hospice is also proud to announce its partnership with the We Honor Veterans program offered by the National Hospice and Palliative Care Organization (NHPCO) and the Department of Veterans Affairs (VA).  We Honor Veterans is designed to empower hospice professionals to meet the unique needs of dying veterans.  The program focuses on respectful inquiry, compassionate listening and grateful acknowledgement to provide comfort to patients with a history of military service and possibly physical or psychological trauma.  Saint Jude Hospice will become a Level 1 partner with We Honor Veterans in April 2011.  For more information on the We Honor Veterans program, please go to http://www.wehonorveterans.org/.

Dear Sue Beam, Marketing Director, Tradition

I wanted to thank you for the wonderful dinner you provided for our family.  Inviting Mom & Dad to meet some of the residents before they move in was a great idea.  The food was excellent and the service was outstanding.  I think the best part of the evening was watching Mom and Dad come out of their shells.  They have not been able to get out and socialize much since Dad’s fall.  Being around so many nice people, they felt so welcomed.  I truly wish we would have done this sooner.  I am looking forward to their Anniversary Party.  Thank you again for offering to host it at Tradition.  They will be moved in, and I can’t think of a better place to have the celebration.  Thanks again for everything!

Emerald Pointe is a Multi-Family apartment complex located right in the Jewel of New Hope.  We have large one and two bedroom apartments with large windows to let lots of sunshine in.  Looking out the window to the court yard you feel that you are on the 14th hole of the rolling hills golf course.  We offer wood burning fire places in our second and third floor apartments and walk out patios on our first floor apartments.  We have a variety of on going activities, such as Pastry Mondays, popcorn Fridays, Celebration of Birthdays, weekly bible study, and an after school program offered to the children.  We had an awesome turn out for our Shamrock Shakes and treats for St. Patrick’s Day and are looking forward to the fashion show that the After School Program will be putting on April 15th.

If you would like to come and visit Emerald Pointe, please feel free to contact Sue or Jamie at 763-546-2018.  We look forward to meeting you.

Bluestone Physician Services has provided superior, on-site medical care to residents at Lang Nelson facilities for some time. We often receive questions on how our practice works. We work as a team of Family Practice Physicians and Physician Assistants/Nurse Practitioners to provide high quality and compassionate medical care to individuals who have difficulty getting out to in-office doctor appointments. Our medical providers partner closely with the residential nursing staff in order to provide easy and timely access to process orders and attend to health care concerns. Nurses and families have access to Bluestone’s secure, on-line website in order to provide regular communication to the medical provider team.
We work differently than the usual doctor’s office in that we see our patients monthly with the goal of improving and/or maintaining health and decreasing the severity of medical issues in order to reduce unnecessary ER visits and hospitalizations. Monthly visits allow us to better understand our patients’ needs; and it allows us to work closely with the nursing staff to provide patient education and training tools in order to maintain a superior living environment for their residents. Each monthly visit includes: addressing current medical concerns, chronic disease management, thorough medication review, foot care (including toenail trimming), skin care, communication and education to the nursing staff and families, and coordination of specialty care services.
As an added benefit, Bluestone Physician Services has both a staff Neurologist and Psychiatrist who are highly experienced in Geriatric Care and are also available for on-site consultations.

More information is available at www.Bluestonemd.com or by contacting any of the Lang Nelson Assisted or Catered Living communities www.langnelson.com.

There’s always a reason to Celebrate at Broadway Village!!

As we watch the snow melt, we long to see the flowers that surround our coffee house, and grilling/picnic areas.  Until then, we all are staying busy with the many activities on and off campus.

We are very proud to announce our new activities director Chad Larson.  Chad loves to cook and bake, and has also won awards at the Minnesota State Fair for his canning and baking.  He has made such an impact on our community already. (Rave reviews on his Gumbo at our Mardi Gras party!)

Say “Elaine” at The Heathers Estates and everybody knows who that is. “Elaine” is synonymous with “Activities”. She has been the Activities Director since the beginning of Time for The Heathers Estates, formerly the Lamplighter Apartments. Elaine moved in almost 30 years ago in 1981. When she became the official Activities Director, it’s hard to say.  She is one of the very first people you will meet when you move into The Heathers Estates. She has her finger on the pulse of all of our transportation. She knows the Target van schedule, the Wal Mart van schedule, and the Grocery van schedule and who the “regular” riders are, like the back of her hand. The sign up sheets go up like clockwork with fancy backgrounds to boot. She doesn’t actually make the coffee, nor drink it, but she knows who’s making it for the event. She is a natural born organizer and people person. Elaine works her magic and brings in our entertainment. Just in this last year, The Heathers Estates has enjoyed MN Orchestra Violinist Tai Chi Chen and his wife Robin, the Metro Men Singers, Vocalist Bob Clark, Keyboardist James Shaw, Accordionist Mark Stillman and much more.

Now known as the Nurse Liaison Program, Lang Nelson’s “Betty” program began with our Elaine when she won a contest with her caring personality. She will write a poem using the letters of your name at the drop of a hat and recite the poem to any audience who will listen.  And she has her own “people”, volunteers, who she calls on to deliver messages, notes and announcements at her every whim.

Elaine grew up in North Minneapolis and graduated as a teacher in 1954. After teaching in the suburbs of Cleveland, OH and Tucson AZ, she worked in the Robbinsdale School District for over 30 years. It gives her great pleasure to run into her former students and their families still in this area. Since her retirement, she enjoys volunteering for Prism and is the instigator for the NEAR Food Shelf. She also enjoys being part of the Story Theatre for children and is the Heathers liaison for the nearby Neill Public Elementary School coordinating hats and mittens for the kids’ winterwear.  Ask her about the Grandparent project for her Neill kids and you will get a treatise on how much love the kids need and how you can help.  It gives her a good feeling working at the Heathers Estates because the people are so pleasant and caring. She is happy for the residents when they are enjoying the fruits of her labor at her home. Elaine is our official Activities person and unofficial mother hen at The Heathers Estates, worrying about her brood of chicks, including office staff, the residents, and the kids next door.
